Did you know the average American spends 8 hours each month coordinating their care?

Value-based care (VBC) is inevitable, but many hospitals and health systems are struggling to survive in a fee-for-service world and lack the resources to invest in true ambulatory care transformation. Overcoming shrinking margins and rising costs usually means increased workloads and reduced spending, leading to overworked providers and worsening patient experience.

Thankfully, there is a way for health systems to immediately improve patient access and address provider burnout while subsidizing the transition to VBC. In this webinar, we’ll provide a roadmap to VBC-readiness that attendees can learn from and follow, with insights from the administrator of an independent hospital that successfully improved care and operational sustainability across its entire ambulatory care network.

We'll cover:

  • How fee-for-service programs such as CCM and APCM can fund foundational infrastructure for VBC
  • How health systems can scale care management while maintaining independence and financial sustainability
  • Practical strategies for leveraging today’s reimbursement incentives to build tomorrow’s VBC capabilities
