Quality Care For Hundreds of Patients — With 91% Average Monthly Engagement

Comprehensive family medicine practice Regional Family Medicine serves patients in a large area of North Central Arkansas and Southern Missouri. Physician leaders at the practice have long been committed to increasing patient access to high-quality, cost-effective care for these communities — especially a large population of older patients managing multiple chronic conditions.

Managing partner Lonnie Robinson, MD, FAAFP thought a Chronic Care Management (CCM) program could help support patients between visits and close care gaps. Initially, he was skeptical about whether his practice could effectively run a CCM program at scale. But he overcame that skepticism once he saw rapid adoption — enrolling 500 patients in just a couple of days. Dr. Robinson will explain:

  • How his practice delivers meaningful care management to over 600 patients (with just 2 FTEs, paid for by the program)
  • How high-quality care management improves chronic condition management—and why meaningful care management is more than just a check-the-box phone call
  • Why patient satisfaction is critical to program success
  • Why CCM can help a practice survive and thrive, buffering the challenges of launching value-based models with predictable monthly income

Join this live webinar and Q&A session to learn how a sustainable care management program can offer clinical value for your practice and your patients.
